If you are a new patient starting service, please register and follow the instructions below:
Once registration is completed I will send you a Welcome Email and request you complete the following attached forms:
Biopsychosocial Form
Counseling Agreement Form
Authorization to Disclose Form (if needed)
Please review the Client Privacy Measures found in the Client Portal in the “Forms” section.
During our first session we will review your or your child's presenting concerns and develop an individualized treatment plan to fit your child’s and/or your treatment needs. I will send the completed treatment plan to you for you to review and electronically sign. Your and/or your child’s treatment plan will be securely sent to you via the CounSol.com/client portal email.
We will set up your and/or your child’s next appointment at the end of your sessions unless otherwise indicated.
